Crossroads Adult Home, 120 Gifford Street, Syracuse, is licensed by the New York State Department of Health as a Congregate Care Level III Adult Care Facility. Crossroads offers single and double occupancy rooms in a supportive residential setting. Staff members provide assistance with daily living, including around-the-clock supervision, case management services, three daily meals and two daily snacks. Social and recreational activities are also available. There is a physician on-site once a week for those without a primary care physician. Trained staff provide 24-hour assistance with medications. Funding can be covered by supplemental security income (SSI), supplemental security disability (SSD), public assistance, social security or private payment.
A case manager is assigned to each resident to help assess his need for daily living supports. Each residents are responsible for personal hygiene, keeping his room clean and doing his laundry. Assistance is provided as appropriate, but each person’s highest level of independence is encouraged.
Crossroads offers an environment where residents can work through their personal challenges and receive support, respect and encouragement.
Crossroads Adult Home, 120 Gifford Street, Syracuse, is a 59-bed supportive residence for men age 21 and older who find daily living a challenge due to mental, physical or developmental disabilities. Residents stay anywhere from six months to the rest of their lives as they reach their greatest level of independence in an environment free of drugs and alcohol.
